Purpose: The Nurse Navigator coordinates healthcare services for patients and serves as both an educator and advocate for those with complex health conditions. Through shared decision making and personalized goal setting, Nurse Navigators develop a plan, in conjunction with the Primary Care Provider, to optimize the patients overall health and wellbeing. Nurse Navigators communicate effectively with patients and their families to develop trusting relationships and encourage patient and family engagement with the plan of care. As a member of the care team, the Nurse Navigator recommends screenings and follow?up care using evidence?based medical guidelines. Nurse Navigators work collaboratively with other departments, both clinical and administrative, to ensure patients receive high quality, comprehensive medical care. Nurse Navigators carry out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization.

Qualifications
Education and Experience: Graduate of accredited nursing school and one (1) or more years working in direct patient care in clinical environment; or a Bachelor of Science in Health Education with one (1) or more years of experience working in direct patient care.
Preferred: BSN degree and experience in case management, home health, or administrative experience. One or more years working at ARC. Spanish speaker.
Knowledge, Skills and Abilities:
Certificate/License: Licensed Registered Nurse in the State of Texas. Basic Life Support Certification (BLS).
